An applicant shall:
- Obtain a Recommendation Letter or Letter of Support from the Ophthalmology Chairman or Head of Department at his/her practicing institution;
- Complete the prescribed application form obtained from the ACS Website or Secretariat;
- Provide detailed CV and attached all relevant certificates;
(e.g. Fellowship Certificate, Educational Certificate) - Provide a letter justifying your choice of ophthalmic institutions with specification of training objectives and areas of interest. This letter should be addressed to ACS-Santen Asia Educational Grant Selection committee members;
- Letter of Acceptance of Observership from the training ophthalmic institution, indicating the duration of the observership;
(Condition: Minimum 2 weeks and Maximum 4 weeks)
* Applicants are required to notify their choiced training institutions if they wish to withdraw from the observership training should their funding request be unsuccessful.
